New Bill Could Save 200 Rural Emergency Rooms Nationwide
Senator Josh Hawley introduced legislation to rescue America's struggling rural hospitals, offering guaranteed funding to keep emergency rooms open in communities where residents sometimes drive an hour for lifesaving care. The bill could protect hundreds of hospitals from closure while ensuring rural families get the emergency medical care they deserve.
Nearly 200 rural hospitals have closed since 2005, leaving families in small towns struggling to access emergency care when they need it most. Now new legislation could turn that troubling trend around.
Senator Josh Hawley unveiled the Rural Hospital Emergency Room Guarantee Act on Wednesday, a plan designed to keep rural emergency rooms from shutting down at what he calls a "breakneck pace." The bill would create a dedicated 10-year funding stream to help struggling hospitals stay open.
The crisis hits close to home in Missouri, where 12 rural hospitals with emergency rooms have closed since 2014. Nearly half of all rural hospitals in the state currently operate at a loss, with roughly 20% at immediate risk of shutting their doors for good.
Under the proposed legislation, eligible rural hospitals would receive a guaranteed baseline of $1 million per year to cover emergency room operating costs. The funds would arrive quarterly and adjust for inflation, giving hospitals the financial stability they desperately need.
Hospitals would also qualify for additional funding based on their specific challenges, including geographic isolation, financial hardship, and patient injury rates. Those facing immediate closure could receive a one-time emergency payment of up to $250,000 to keep operating while they stabilize.

The funding would come through the Health Resources and Services Administration. Hospitals receiving these guaranteed funds would maintain their eligibility for other federal healthcare programs, grants, and designations.
The Ripple Effect
When a rural hospital closes, the impact spreads far beyond emergency care. Residents in affected areas often face hour-long drives to reach the nearest emergency room, turning manageable health crises into life-threatening situations.
But this legislation represents something bigger than keeping buildings open. It signals recognition that rural Americans deserve the same access to emergency medical care as their urban counterparts.
The bill comes as President Trump announced a $50 billion rural healthcare plan focused on expanding price transparency and lowering drug costs. Together, these initiatives show growing momentum around solving rural healthcare challenges that have persisted for decades.
For the roughly 60 million Americans living in rural communities, guaranteed access to emergency care could mean the difference between life and death. This legislation offers a pathway to ensure that when medical emergencies strike, help is never more than a short drive away.
