Regional Australia Doubles Bulk-Billing Clinics in One Year

When Matthew Lillywhite moved to Canberra in 2023, a standard doctor's visit cost him $100 out of pocket. Within three years, that price jumped to $140.

The public servant lives with chronic health conditions requiring regular monitoring. A flight attendant friend helped him access $80 return tickets to Melbourne, where his family doctor still bulk-billed at no cost.

"Canberra doesn't feel like it has universal health care anymore," Lillywhite said. He now helps classmates afford medical costs and drives them to distant bulk-billing appointments they'd otherwise miss.

But new data from online GP directory Cleanbill shows a promising shift. Regional and remote areas are driving a dramatic surge in bulk-billing across Australia.

The April 2026 Blue Report, analyzing 6,900 clinics nationwide, found the percentage of fully bulk-billing clinics almost doubled in one year. The rate jumped from about one in five to just over two in five clinics.

Regional centers, large and medium rural towns, and remote communities all recorded bigger jumps than metropolitan areas. Metro areas saw the lowest percentage increase at 16.8 percent.

The change means more Australians can see their doctor without paying anything upfront. Federal Health Minister Mark Butler said more than 3,700 medical practices now fully bulk-bill, with 1,400 previously charging mixed fees.

About 97 percent of Australians now live within a 20-minute drive of a bulk-billing practice, according to Butler.

The Bright Side

While out-of-pocket costs rose 13.5 percent for those still paying fees, the bulk-billing surge shows the system responding to community needs. Regional areas that historically struggled with healthcare access are leading the charge toward more affordable care.

The shift matters most for people like Lillywhite, who juggle chronic conditions on limited budgets. When one clinic in a small rural town switches to bulk-billing, it can transform access for an entire community.

Lillywhite's increased salary now helps him afford care and support others who can't. "If you can't afford to pay upfront, your options shrink really quickly," he said.

The Royal Australian College of General Practitioners noted bulk-billing increases where funding better reflects the true cost of care. The trend suggests sustainable change may be taking root.

More Australians can now walk into their local clinic and leave without opening their wallet.
