Rural Cancer Patients Thrive with Surgery Close to Home

Rural Americans with cancer may no longer need to face grueling trips to distant city hospitals for surgery, thanks to promising new research from the University of Louisville.

A study of more than 16,000 Medicare patients with colon or lung cancer found something remarkable. Those who had surgery at their local rural hospitals did just as well as patients who traveled to big urban medical centers.

Dr. Michael Egger and his team analyzed outcomes for patients living in rural areas across the United States. More than half of colon cancer patients and a quarter of lung cancer patients chose to have their operations close to home.

The results were reassuring across the board. Survival rates, complication rates, and hospital readmission numbers were nearly identical whether patients stayed local or traveled to urban facilities. At 90 days after surgery, mortality rates were about 5% for lung cancer patients and 7% for colon cancer patients in both groups.

The travel difference was substantial. Colon cancer patients who went to urban centers drove about 33 extra miles and added 35 minutes to their journey each way. Lung cancer patients traveled an extra 26 miles and 23 additional minutes.

For rural families already facing cancer's emotional and financial burden, those extra hours and miles add up quickly. Multiple appointments mean time off work, childcare arrangements, and mounting gas or lodging costs.

Dr. Egger emphasized that some patients may still need or prefer specialized urban centers for their care. But this research offers preliminary evidence that many complex cancer surgeries can be performed safely closer to home.

The Ripple Effect

This discovery could reshape how America delivers cancer care to its 46 million rural residents. As hospital systems increasingly centralize services in cities, this research suggests a different path forward.

Strong surgical outcomes at community hospitals mean rural medical centers can continue offering vital services. That keeps healthcare jobs in small towns and preserves the local expertise that serves these communities year-round.

The research team is now studying what makes certain rural hospitals succeed at complex cancer care. Their goal is to identify best practices that can help more community hospitals achieve excellent outcomes.

Future research will also examine the full cancer journey beyond surgery, including chemotherapy, radiation, and follow-up care. Understanding where gaps exist will help close the health disparities that rural Americans face.
