Stem Cell Transplant Cures Rare Autoimmune Disease for 15 Years

Two people given months to live have been symptom-free for over 15 years thanks to a stem cell transplant that reset their entire immune systems.

The man and woman both suffered from neuromyelitis optica spectrum disorder, a rare autoimmune disease where the body attacks the spinal cord and optic nerve. Episodes of eye pain, vision loss, vomiting, and paralysis made normal life impossible, and standard medications couldn't help them.

In 2009 and 2010, doctors at IRCCS San Raffaele Hospital in Milan tried something never attempted before for this disease. They gave each patient stem cells from a healthy donor, completely replacing their malfunctioning immune systems.

The results published in Med tell an incredible story. The man, who received cells from his sister, recovered so fully that he resumed normal life and became a father to two children. The woman gained back the use of her arms and no longer needs any medication.

Before the transplant, both patients underwent chemotherapy to wipe out the harmful immune cells producing the attacking antibodies. Then they received a single infusion of donor stem cells, along with drugs to prevent the new cells from attacking their bodies.

The Ripple Effect

This breakthrough opens doors for thousands suffering from severe autoimmune diseases that don't respond to standard treatments. Unlike therapies using a patient's own stem cells, which only reset the immune system, donor cells completely replace it. This difference matters enormously when the original immune system is fundamentally broken.

Neither patient developed the dangerous complications common with stem cell transplants, and both built healthy new immune systems from scratch. Researchers say the 15-year remission period suggests the treatment addressed the root cause rather than just managing symptoms.

Scientists at the University of Technology Sydney call keeping people symptom-free for this long "exciting." While researchers hesitate to use the word cure, both patients have lived more than a decade and a half without the disease that once controlled every aspect of their lives.

The success has convinced the medical team that this experimental therapy deserves a much larger clinical trial, potentially helping countless others trapped by autoimmune diseases that steal their futures.
