Arkansas Covers Doula Care for Medicaid Patients

Thousands of Arkansas parents will soon have access to professional birth support they couldn't afford before. Starting September 1, Medicaid in Arkansas will cover doula services for low-income expectant families.

The Arkansas Legislative Council approved the coverage rules on Friday, making the state's 2025 law requiring insurance to cover doula care a reality. Doulas provide emotional, physical, and educational support during pregnancy and childbirth, acting as advocates between parents and medical staff.

Madison Jones of Cabot knows firsthand what difference that support could make. When she gave birth to her first child in 2024, she couldn't afford a doula and struggled to advocate for herself during delivery.

"It's really hard to speak for yourself when you're in the middle of a huge transition, a painful transition at that," Jones said. She's expecting her second child in December and looks forward to having that extra voice in the delivery room.

The timing couldn't be more critical for Arkansas. The state consistently ranks among the highest in the nation for maternal and infant mortality rates.

Rural families face additional challenges, with 53 of Arkansas' 75 counties lacking hospitals that deliver babies. Research shows longer drives to delivery hospitals increase health risks for both mothers and newborns.

Annie Hill runs the Bright Blessings doula service in Cabot and plans to enroll as a Medicaid provider. While the reimbursement won't be highly profitable for her business, it will dramatically expand who she can serve.

"I'm incredibly excited that we're this close to putting it into practice," Hill said. She's already spreading the word to expectant parents about the new coverage.

The state is working to enroll certified doulas quickly so families can access care starting September 1. The Department of Human Services says it can typically complete provider enrollment in 15 days or fewer.

The Ripple Effect

Biliah Muhammad owns an indoor playground and is telling low-income parents among her customers about the new benefit. She was on Medicaid when she gave birth in February and wishes Hill's services had been available to her then.

Now she's making sure other mothers know help is within reach. "That help wasn't available to me, and now it is, and we can spread the word and help other mamas who can't afford it," Muhammad said.

The coverage came from recommendations by a task force created by Governor Sarah Huckabee Sanders in 2024 to improve maternal healthcare across the state. What started as a policy discussion has become a lifeline for families navigating one of life's most vulnerable moments.
