
New Breast Scan Cuts Unnecessary Biopsies by 25%

Every year in the United States, doctors perform more than a million breast biopsies to check suspicious lumps. The anxiety-inducing reality? Between 75% and 80% of those biopsies turn out to be benign.
Now, researchers at Washington University in St. Louis have proven a new imaging method can reduce unnecessary biopsies by nearly 25% while missing fewer than 2% of actual cancers.
The technology, called ultrasound-guided diffuse optical tomography, shines near-infrared light through breast tissue to create 3D images. It measures two key signs that reveal whether a lump is dangerous: the concentration of hemoglobin (which cancerous tumors pack with extra blood vessels) and oxygen levels (which drop in malignant tissue as tumors consume it).
Dr. Quing Zhu, who pioneered the technique, and Dr. Debbie Bennett, chief of breast imaging at Barnes-Jewish Hospital, led a double-blind trial with 226 patients. Radiologists first evaluated each patient using standard ultrasound alone, then reconsidered their biopsy decision after seeing the optical imaging results.
The light-based scans revealed stark differences. Malignant tumors showed significantly higher hemoglobin levels and lower oxygen saturation than benign lumps. The technology even distinguished between cancer grades, with the most aggressive tumors showing the clearest signals.

Out of 226 patients, doctors found 77 cancerous lesions, nine high-risk lesions, and 140 benign masses. Adding the optical imaging to standard ultrasound evaluation would have safely eliminated nearly a quarter of the benign biopsies.

The research, published in Breast Cancer Research, represents the largest study of its kind in the diffuse optical imaging field and the first to involve multiple radiologists in a rigorous double-blind format.
