South Africa Maps Breast Cancer Care to Drive Change

South African researchers have achieved something that's never been done before: a complete national map of breast cancer care that shows exactly where the gaps are and how to fix them.

Dr. Laurie Milligan and her team at the University of Cape Town analyzed data from 43 hospitals across all nine provinces, creating the first comprehensive snapshot of who has access to what care. For the first time, health officials have concrete data showing which communities need urgent support and which successful programs could be replicated elsewhere.

The study, published in the South African Medical Journal, reveals that basic breast cancer surgery is available at most facilities, with the majority able to perform breast-conserving procedures. When patients reach surgical care, the average wait time of 28 days falls within national guidelines.

But the research also exposed critical gaps. Only half of hospitals can perform sentinel lymph node biopsy, a less invasive procedure that reduces complications. Just 35% offer breast reconstruction, and advanced diagnostic tools like MRI and PET scans remain scarce outside major urban centers.

The most urgent finding: around 67% of patients arrive with advanced-stage disease, suggesting the real bottleneck happens before they ever reach specialist care. Patients often make multiple visits to primary healthcare facilities before getting appropriate referrals, with months passing between first symptoms and treatment.

What makes this research powerful isn't just identifying problems but creating the evidence base that policymakers need to act. South Africa already has a Breast Cancer Prevention and Control Policy with clear guidelines for specialist breast units and standardized care, but those plans have languished without implementation data.

Now, for the first time, officials know exactly which provinces need more specialists, which facilities require diagnostic equipment, and where early detection programs could save the most lives. The study provides measurable targets and identifies successful models that can be scaled.

"There is no shortage of policy," Milligan noted. "The challenge is implementation." This research removes that excuse by showing precisely what needs to happen and where.

The team's findings on early detection offer particular hope. Because most delays occur before patients reach surgical care, improving referral systems and primary healthcare screening could dramatically change outcomes without requiring expensive new infrastructure.
