South Africa's First Generic Weight Loss Drug Arrives

A lizard living underground in North American deserts just helped make life-changing medication affordable for millions of South Africans.

In August, Sun Pharma launched South Africa's first generic semaglutide injection, the same ingredient that powers blockbuster drugs like Ozempic and Wegovy. With 13 more generic applications under review, prices for these game-changing medications are finally starting to drop.

The story behind these drugs reads like science fiction. In the 1990s, American scientist John Eng discovered something remarkable in Gila monster venom. The reptile emerges from darkness only a few times yearly to eat, yet maintains steady blood sugar between feasts.

Eng found a molecule in the venom that mimics a human hormone called GLP-1. It slows digestion, creates feelings of fullness, and steadies blood sugar levels. He suspected it might help people with diabetes, and he was right.

That discovery paved the way for Ozempic, which launched in 2017 for diabetes treatment. People soon discovered it worked brilliantly for weight loss too. By 2025, the GLP-1 market was worth enough that analysts expect it to reach $190 billion by 2035.

But there's been a catch. These medications cost between R3,000 and R10,000 monthly in South Africa. Even people earning R1 million yearly struggle to afford them.

The high prices created a dangerous black market. People bought from unlicensed sellers or tried experimental drugs still in trial phases. One former professional athlete gained 30kg after trauma and antidepressants, then couldn't find Ozempic due to global shortages.

She turned to a compounding pharmacy charging R2,500 monthly. Within days, constant hunger disappeared. She lost 10kg and started boxing again.

The Bright Side

Now that semaglutide patents expired in March, generics are flooding the market. Competition means lower prices, which means more people can access medication that genuinely changes lives.

The former athlete's experience shows why this matters beyond vanity. She became pre-diabetic and tried everything before finding GLP-1s. For people struggling with diabetes, obesity, or weight gain from necessary medications like antidepressants, these drugs offer real medical solutions.

Generic versions won't just undercut black markets. They could eventually reach public-sector patients who need them most but couldn't dream of affording brand-name versions.

Sun Pharma's generic is just the beginning. With over a dozen more applications pending approval, prices should continue dropping as competition increases. The medication born from lizard venom and scientific curiosity is finally becoming accessible to everyday South Africans who need it.
