Basque Country Cuts Colon Cancer Deaths in Half

A screening program in Spain's Basque Country has cut colon cancer deaths in half among people aged 50 to 69, proving that a simple test can save thousands of lives.

The program uses a quick, non-invasive test that checks for hidden blood in stool samples. Offered every two years to adults between 50 and 69, the test catches cancer before symptoms even appear.

Researchers tracked nearly 2.2 million people over 21 years and found that death rates in the screened age group dropped from 39.7 to 19.8 per 100,000 people. That's a 50% reduction that has held steady for more than two decades.

The turning point came just three years after the program launched in 2009. By 2012, researchers could already see death rates falling, and they've kept dropping ever since.

Dr. Luis Bujanda, who led the study published in JAMA Network Open, estimates that without the screening program, deaths would have been 46% higher. The numbers speak clearly: early detection works.

The program detected 4,892 cases of colorectal cancer during the study period. More than seven in ten were caught at stages I or II, when treatment is far more effective and less aggressive.

When the test shows hidden blood, patients get a colonoscopy to confirm the diagnosis. Doctors can even remove precancerous polyps before they turn into tumors, preventing cancer altogether.

Colorectal cancer ranks among the most common cancers and leading causes of cancer death worldwide. But the Basque Country proves that organized screening programs can dramatically change those statistics.

The success has implications far beyond one region in Spain. Public health officials now have concrete evidence that systematic screening saves lives and catches cancer when it's most beatable.

The research reinforces what doctors have known for years: catching cancer early dramatically improves survival rates. But it also shows that making screening accessible and routine transforms those possibilities into reality.

While the study shows a strong association rather than definitive cause and effect, the scale and persistence of the decline over two decades make a compelling case. When people get screened regularly, fewer die from a disease that's often preventable.
